Apogee St. Croix – Camp Waiver and Release of Liability

By signing below, I understand and agree to the following:

Assumption of Risk: I acknowledge that participation in Ninja Warrior Camp involves physical activity and building projects that carry inherent risks, including the risk of injury. I assume full responsibility for any such risks.
Medical Care: In the event of illness or injury, I authorize the staff of Apogee St. Croix to seek appropriate medical care for my child. I understand that I am responsible for any associated costs.
Supervision and Behavior: I understand that my child is expected to follow directions, respect others, and participate in camp activities in a safe and responsible manner. Disruptive or unsafe behavior may result in early dismissal from camp without refund.
Liability Release: I release and hold harmless Apogee St. Croix, its staff, and volunteers from any and all liability for injuries, damages, or loss sustained by my child during camp.

Release of Media

I give permission for my child to appear in camp photos/videos used for Apogee St. Croix promotions.

I do not give permission.
